前　　言
临床中药学是研究中药临床合理应用的一门学科。药物应用的合理性主要反映在提高药物疗效和降低其毒副作用两个方面，以确保用药的有效性与安全性。本书收载常用中药433种，比第二版增加15种。按药物主要功效分列20章。每一药物条目分异名、基源、成分研究、药理研究、炮制研究、性味归经、功效主治、临床应用、用量用法、制剂与成药、不良反应、中毒救治、使用注意等13个方面。其中，临床应用部分又细分为单方验方、配伍应用、鉴别应用三个部分。中药临床应用有三个特点：， 中药很少单味使用，多以复方形式出现，这里涉及药物的配伍。第二，中药材大多须经炮制后始能入药应用。药物经过炮制，可以增强疗效，或降低毒副作用，或改变药物性能，以适应病情需要，或改变药物某些性状，便于贮存、制剂和服用。第三，临床应用中药都是在中医药理论指导下，强调辨证论治。以上三个特点可以概括为三个关键词，即：配伍、炮制和辨证用药。(1)配伍应用《神农本草经》序例中提到：“药有阴阳配合，子母兄弟……有单行者，有相须者，有相使者，有相畏者，有相恶者，有相反者，有相杀者。凡七请，合和当视之，相须、相使者良，勿用相恶、相反者。若有毒宜制，可用相畏、相杀，不尔，勿合用也。”这段文字概括了药物七种配伍模式，即后世所称的药物配伍“七情”。但具体药物的配伍，则需要依靠长期临床实践经验的积累和总结，集中体现在前人留下的方剂中。本书在每一药物条目中设“配伍应用”，专栏，以“对药”形式，将展示该药物通过不同配伍体现其不同的功效特点，并将文献或方剂出处尽可能附于每一条目后，以便读者查阅。但前人积累的这些宝贵经验，仍然需要在现代临床实践中去求证、优化、发现和创新，以期进一步提高临床疗效和增强用药的安全性。(2)鉴别应用临床合理应用中药涉及如何鉴别应用的问题。例如同名中药但属不同植物基原,或者同一植物基原但入药部位不同，或者功效相似但毒性强弱有无不同，以及同一药物经不同炮制后的成品等，临床如何鉴别应用。为此，本书在每一药物条目中专设“鉴别应用”栏，论述不同药物的性能特点和适应病证，防止误用或任意相互替代使用，以免造成不良后果。(3)单方验方本书收录了具有实用价值的古代验方，更多地收录了现代临床报道的有效处方。其中单方较少，大多为复方，符合目前临床用药的实际情况。所治疾病均用现代病名表述，采用病证结合的方式可能更能为广大读者理解和接受。所谓验方，乃是在中医理论指导下通过辨证论治获得一定疗效的处方。用辨证论治和病证结合的认识观来看待验方,体现了中医对疾病诊治认识论和方法论的有机统一。（4）不良反应药物普遍具有药性和毒性两种性能。因此，药物对人体而言不仅有治疗作用，若应用不当还可能产生有害作用。过去，人们往往专注于中药的药性，而疏于对毒性的认识。本书对中药临床应用中可能产生的毒副作用以及其他不良反应的文献报道尽可能给予收载，以引起临床工作者的重视，在应用中尽量趋利避害，保证用药安全。对于前人配伍十八反、十九畏，妊娠用药禁忌等内容，则予以保留，放在“使用注意”一栏中，同时将现代中药研究涉及用药安全的相关知识，以简明提示的方式，也一并纳入其中。中药的合理应用，前人主要依靠长期的临床实践经验的积累和不断总结，并载入历代中医药典籍中，成为国家传统医药文化的瑰宝，其合理用药的经验至今仍有非常高的实用价值。然而，中药的合理应用仅仅停留在临床经验层次上显然还是不够的。中药的现代基础研究已成果累累，主要反映在化学成分研究、药理研究、炮制研究三个方面。因此本书在每一中药条目中特别增设上述三个栏目，为读者提供合理用药的新思路、新方法，并为诠释中药的药性和毒性提供现代科学理论支撑，使临床用药的有效性和安全性得到进一步保证。此外，本书设“制剂与成药”一栏，为临床工作者提供多种药物制剂的应用选择。由于本书内容已并不完全是传统意义上的临床中药学，故取名《现代实用临床中药学》，以示区别。本书在编写过程中，曾得到中国药科大学严永清教授、南京中医药大学宋立人教授的悉心指导，在此表示衷心的感谢!化学工业出版社对本书的出版给予了大力支持，在此一并表示衷心的感谢!鉴于本书编写是一种新的尝试，涉及的学科领域十分广泛，因此在编写内容的深度、编写体例上有不尽如人意之处，祈望读者批评指正，以便日后修正。中国药科大学 祁公任2017.12
